چکیده

Introduction Although there is a paucity of randomized trials evaluating outcomes after primary-prevention implantable cardioverter-defibrillator (ICD) placement for patients with hypertrophic cardiomyopathy, guidelines recommend ICD placement for certain patients with the disease. Apical hypertrophic cardiomyopathy (AHCM) is one pattern of disease manifestation with unique characteristics. Hypertrophy involves predominantly the apex of the left ventricle and at times the right ventricle. Whereas some have suggested a benign course of disease in cases of AHCM, there are a few reports of patients developing sustained ventricular arrhythmias and sudden cardiac death. We present a case of ventricular fibrillation (VF) and ICD failure because of sensing failure and subsequent ICD manipulation in a patient with AHCM. This case brings to light longitudinal changes in the hypertrophic tissues, electrical changes during VF, and the need for interval testing to ensure proper device function in the setting of tissue remodeling over time.

Apical hypertrophic cardiomyopathy with hemodynamically unstable ventricular arrhythmia - Atypical presentation.

We present a patient with asymptomatic apical hypertrophic cardiomyopathy (AHCM) who recently developed cardiac arrhythmias, and shortly discuss the diagnostic modalities, differential diagnosis, and treatment strategy for this condition. AHCM is a rare form of hypertrophic cardiomyopathy, which usually involves the apex of the left ventricle. AHCM can occur with varied presentations such as ch...

Implantable cardioverter-defibrillator in a patient with dextrocardia situs inversus

Background: Dextrocardia is a congenital anomaly, which may have coexistent coronary artery disease (CAD), arrhythmias and conventional indications for device therapy. However, the implantation of transvenous leads can be technically challenging and the approach needs to be tailored to the patient's individual anatomy. Case presentation: A 54-year-old male with dextrocardia situs inversus and i...

Hypertrophic cardiomyopathy: role of the implantable cardioverter-defibrillator.

OBJECTIVES We report the occurrence of cardiac events during long-term follow-up in patients with hypertrophic cardiomyopathy (HCM) after cardioverter-defibrillator implantation. BACKGROUND The identification of patients at high risk for sudden death and the prevention of recurrence of sudden death in HCM represents a difficult problem. METHODS We retrospectively analyzed the occurrence of ...

Mid-Septal Hypertrophy and Apical Ballooning; Potential Mechanism of Ventricular Tachycardia Storm in Patients with Hypertrophic Cardiomyopathy

Medically refractory ventricular tachycardia (VT) storm can be controlled with radiofrequency catheter ablation (RFCA), however, it may be difficult to control in some patients with hemodynamic overload. We experienced a patient with intractable VT storm controlled by hemodynamic unloading. The patient had mid-septal hypertrophic cardiomyopathy with an implantable cardioverter defibrillator (IC...

Delayed AICD therapy and cardiac arrest resulting from undersensing of ventricular fibrillation in a subject with hypertrophic cardiomyopathy-A case report.

Defibrillation testing is no longer routinely performed after automatic implantable cardioverter-defibrillator (AICD) implantation. However, certain subjects undergoing AICD implantation may be at higher risk of undersensing of ventricular arrhythmias resulting in potentially fatal outcomes. We present the case of a 30-year-old woman with hypertrophic cardiomyopathy (HCM; 'asymmetric septal hyp...
